  • We accept most major insurance plans and we welcome your call to verify our participation with your particular plan.
  • For your convenience, we accept Visa, MasterCard, American Express, Discover and CareCredit (possible to have hyperlink to Care Credit website)
  • As a courtesy we can submit a claim to your insurance company for treatment.
  • We do our best to provide an estimate of your insurance coverage; however, final determination is made once a claim is processed.

Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.

We will send you a monthly statement while there is an outstanding claim with your insurance company. We ask that you take responsibility for resolving outstanding claims with your insurance while making payments on your account. Your prompt remittance is appreciated.
